Hi,
At a conceptual level answer for your first query is provided by the attached document. This is a real "Pronotion Policy" organizations have been following.
By way of an answer to your second query, I would state that a comparison on "industry-cum-region basis" coupled with "capacity to bear additional financial burden" should provide you the guidelines. You may refine the dindings by factors like, "cost/need to retain", "job analysis to determine the worht of the job and then an objective performance appraisal system to decide how well the individual qualifies!"
Promotion is an illusiory concept. An indusrty that is not growing fast, not changing or innovating will by definition offer fewer opportunities to grow! But it is found that many individuals equate higher rank and better pay with equation-unfortunately!!
I hope this answers your queries.
