How DO i write a experience Letter??

I want a sample of how to write a experience Letter. i mean i gotta hand it to my boss, and then he said he will fix it around a bit. i basically work in a pharmacy and i am a student and have been working for 2 years in the pharmacy. I am applying for colleges and i need to give a my boss how i want my exp letter. basically just to let my colleges kno that i have been working for this long and blah blah. and stuff i just dont know how to write it so please if someone can help. PLease> thaNKs

you can say
