We have to inform you that due to the introduction of Bio Metric Cards by ESIC, all existing insured persons are required yourself to fill in the New ESIC Forms for Bio Metric Cards and new appointees are required to fill up ESIC Declaration Form Old and New ( The New Forms of old and new Employees are to be keept ready as & when the ESIC authoritries will demand we have to submit the same) in the format attached. ESIC has introduced this new format for processing Bio-Metric Cards.
Further changes in the Family definition is as under
*. Family will not include parents if the monthly income of the parents is more than Rs. 3500.00
*. For Unmarried Applicant. Family will not include brothers and sisters. Please mention only and only if they are dependent on your income.
*. For married applicants, Family will include only Wife and Children.
