I have recently joined a mid sized broking firm as HR. Can anyone help me with the format of Leave application form.
I would also like to know is there any difference in HR Policy and Employee Handbook.

The following things should be covered in a leave application format:
Date of Application:
Employee Name: ___________________ ID no: __________
Desgn & Depart:
Dates & days of leaves being applied for: From______ to _______(dates)
Reason for leave:
Contact details when on leave:(these details are required incase of emergency).
Immediate reporting authority/boss personnel officer
If you need a copy of the actual form pls mail me ur personal ID i shall send u a scanned copy of the same.
