Hi,
Please can you forward me a questionnaire, we would like to take feedback from all our employee on monthly basis so that we come to know what are the major problems they are facing , how can we improve ourselves , how we can keep them happy, retain them etc .

From what you have written it appears that you want to conduct exploratory research. So just ask them to list 3 points in the order of importance the problems they are facing, if any. What needs doing to eliminate or reduce the problems, etc. Once you have gathered that information, you may be able to prepare a quantitative questionnaire to elicit their responses using Likert scale.
How many employees have you got? Do you know how much it will cost to conduct a monthly survey? Will that be cost effective and can that be justified in this period of impending recession?

I think I was not able to explain what was required. we need to talk with our employee who are working at site , need to know how they are feeling, what problems they are facing at site, any personal problem ......... just want to know them better

Shweta still I am in the dark. Could you kindly tell me (if required by private message) more about your organisation, its size, activities undertaken, the type of workers (qualified or not), etc.
On CiteHr many do not ( or are not, as you have appreciated and acknowledged) explain the scenario and their specific requirements. This makes it extremely difficult for us to give effective answers in an efficient way.

I also suggest that you use the search facility at the top blue bar and slect advanced search and use the key words from your query and see if your question has already been answered in response to someone else's query.

please can you forward me a layout for confirmation letter which specifies that company is confirming an employee but he needs to improve as he is lacking in practical knowledge , needs to get corrective measurement to improve his performance. We would also like to inform him in polite manner incase he will not deliver good company might have to think over it

My dear sweta!!
i would suggest you not copy any questionnaire. you probably get n number of questionaire if you search in net but that may not fulfill your need. try to understand your orgn. culture, your peoples' expectation from orgn. and vise versa. improve your communications through more that one channel then you make your own. obviously you can take the suggestions from your friends.. from this forum.
