this is a forum we can share our ideas ,it is very difficult fill up simple form and circulate it .it is a simple form reqd some personal information,one should know how to fill a form,if ant difficulty can be discussed.

You can collect the forms from the nearest PF office - on production of your pf number.
The forms are normally in English and local languages - it is easy to fill up the forms - nothing to worry.
After filling up the forms you attach a xerox copy of your Savings Bank Account Passbook / statement which depicting the Account number clearly.
