Need for Questionnaire

Hi members,

Iam doing my project work on "Employee Morale" for a Security Management Service Organization. My doubt is should i design my questionnaire for 'Employee Satisfaction Survey' or 'Employee Motivation Survey'. Can anyone send me a sample of the questionnaire.

Please work on the questionnaire and attach it for comments, ie you can pilot it to iron out deficiencies.

Few Sample questions on Employee Morale

1. How happy are you vth your job?
2. How do you feel about the employee - review system?
3. Why do you come to work everyday
4. How do you feel about the training and orientation program you experiences when you started? Do you feel you understand the procedures, policies, and responsibilities that are part of your job? How would you change things?
5. What do you like least about you job and/or the company?

Note:

When you are doing project or survey on Employee Morale, pls consider these two points

- Satisfaction of your employees
- Communications
- Organizational leadership
- Organizational Culture
