Form No. 11 In E.P.F. - Provident Fund

Please tell me about the Form No. 11 in E.P.F.

Whenever an employee joins, the employer is required to take a declaration from the employee as to details of his previous Provident Fund. The same is taken in the prescribed form, form 11.

Plz tell me that-
if any person ( women worker ) worked in a company & give esi contribution from 1 year. But now she left service after giving her contribution for 30 days in a contribution period. now she want meternity benifit from esi , is she eligible for it ?

Tushar, do you mean form 11 or the deviated question about ESI?

Since the woman employee is not in service she can not claim maternity benefit (which means payment equivalent to average salary if she had been in service) from the ESI Corporation. In order to get qualified for maternity benefit she should have worked at least for 70 days during the preceding two consecutive contribution periods and he confinement should have happened during the benefit period.
