I was nice to read your posting. As on this, I would like to know to which domain you do your require the questionare ie. your company's current funational domain area. and the sub domain area for the innovation.
Also mention what specific innivation do u need like- funcational or process based.

My company is a pharmaceutical company where we wanted to know views of our managerial level employees about innovation. it includes what they exactly mean by innovation, how important they feel it is, wheather they are using it in their work or want to do that in short thier attitude towards it.
We want to make questionnaire in such a form that we can quantify the results.
What i am currently working on are the objective type of questions where each option will have points and depends upon the options selected by employee his score can be generate. I want a help in various parameters that we can use in this.
