Thumbs up Company's Survey

My Organization wants to have a survey among the employees to know what they feel about the organization, its policies, etc. Do they have trust in the top management, what are the areas they like about the company, what are areas they think needs improvement for better productivity. Basically the idea is what a employee at junior level thinks, a CEO or COO might never come to know. So please help me out with this kind of survey, suggest questions.
Please do not confuse it with Employee Satisfaction Survey. Its about knowing your employees better. What do they think about the Organization.

Hi Waseem,

We conduct employee surveys for some of the biggest name.

Typically the questions that you need to ask should revolve around the company and its people

1. Working Conditions
2. T & D Options
3. Work progression
4. Superiors Attitudes
5. Subordinate Attitudes
6. Co-Worker Attitudes
7. Views on Policies / R & L
8. Growth path of the company
9. Employee Management Mechanism
10. Feedback

You could certainly elaborate on all of the above.
