Urgently Need Appointment Letter Format

Dear All,

I work in HR Dept. and the current Appointment Letter format which is used in my company needs to be changed.

I am given the responsibility to update the appointment letter.

Can somebody give me a structure of appointment letter which has all necessary points covered... like probation, emoluments, work ethics, confidentiality, transfer, kras, medical requisites, termination, etc.

I am trying to form a letter on my own but it's not up to the mark as I want.

Can somebody please provide me a sample or a format?
