Default Format for Leave application received

Hi all
Just a lit bit info about me i work as a Assist Manager HR and my bad luck its one of the most horible place ive ever seen in my life. There is no HR manager only an Assistant HR manager.

I need some kind of format where i can put the information about Name of the employe, date leave applied, Leave required, Leave available, Leave status, Leave start date, leave finish date etc

Your help will be highly appreciated.
