I have developed a full and final settlement letter, which acts as a certificate as well as a computation slip (payslip for the last month of working). We are currently using this format. I have just added some possible salary heads. This would, ofcourse, vary from firm to firm.
See if this takes care of what you are looking for. I guess, using this slip alongwith a letter stating the details of cheque enclosed or demanding the amount to be recovered would be sufficient

Submit all DNC lists to
Remove their caller ID number from my display
Return all lists that have been dialed with corresponding disposition reports
Once all the above documents were satisfactorily received by the Client payments due will be made for the services rendered to this date.
