Plz find below the Releiving Letter which you can issue. From option a or b whatever is applicable youc can keep in the letter else delete.:--

With reference to your Resignation Letter dated 16th January 2008, the management has duly accepted the same and accordingly you are being relieved from the services of the company as Software Engineer with effect from 14th February 2008.
a) Herewith, attached the cheque amounting to Rs/- , along with the computation sheet towards your Full and Final Settlement.
b) We confirm the receipt of Rs./- by Cheque / DD no.<> dated drawn on against the amount in lieu of Notice Period.

I have developed a full and final settlement letter, which acts as a certificate as well as a computation slip (payslip for the last month of working). We are currently using this format. I have just added some possible salary heads. This would, ofcourse, vary from firm to firm.
See if this takes care of what you are looking for. I guess, using this slip alongwith a letter stating the details of cheque enclosed or demanding the amount to be recovered would be sufficient

Please contact the Accounts/ HR department and settle & make full & final settlement this issue. So that we can issue your reliving letter.
