Queries about appraisal??????????

Hi,

Experts please help me out to get rid of following queries regarding appraisal:

1) can an org. use a single appraisal format for all department or there will be different format for different department?

2) What are the basic rule / procedure for appraisal?

3) Who will do the appraisal for HR? What are the basic guidelines to appriase?

4) How can evaluate the performance?

5) Any other inputs are alos invited.

Actually I am totally in dark to initiate. We are a facility co. (housekeeping co.) department: (Operations / Supervisor / HR / Finance / Site Supervisor)

Hello,

appraisal is the most important part of u r job once u complete more then 6months in a particular company u r appraisal suppose to be done by u r reporting authority or by HR , it is based on ur performance & the out put which u have given to the company with in the 6months till nw ........ every company follow the same procceducre for appraisal ............. ur performance will be rated from 1to5 the HR will rate u r perfomance with in 1 to 5 ...........

I am still confused with my queries, please describe me who will assess HR
