On [DATE], at [TIME] we met to discuss the problem of your unacceptable habit of taking frequent breaks during working hours. Please note that your act influences other team members negatively and thus bringing down the discipline level in our organization.
All you need to do to improve your habit you need to observe break during specified break-timings.
I will assist you in any way that I can to remedy the problem, however, unless the matter can be corrected, I shall have no other alternative but to further disciplinary or corrective actions. Please treat this warning seriously and improve accordingly.
Slight difference here is that the question was related to notifying someone who created indiscipline among the group. And this notification was solely addressed to that particular fellow hence those salutations and closures.
Using them in this way do not change its legal status.
