Which form i have to fill up to complete the formalities for the same.

I have the forms 10c and 19. After filling up the forms, other than the old company, where should i submit the completed forms and how ?
Is there any formalities or rules to follow ? Kindly explain on the same.

Here is the use of these two forms:
Form 10c:
For claiming :
- Refund of Employer share.
- Withdrawl benefit
- Scheme certificate for retention of membership
Form 19:
To be submitted by a member to withdraw his Provident Fund dues on leaving service/retirement/termination.
So these two forms are required to withdraw the PF...OK?..
